• $75/month. Insurance will not be billed, so there are no deductibles or health insurance copays out of your pocket.

Services covered by the monthly fee are:

24/7 direct physician access to the physician via cell phone/email
Quarterly community meetings with the doctor/community health resources
Courtesy visits at the hospital and/or nursing home
Hospice MD care
Same day visits for urgent needs
Typical office visits will be 30-60 minutes.
